                    Monsieur le président, messieurs les ministres, chers collègues, comme vous le savez, mon groupe avait pour priorité sept propositions relatives aux soins orthodontistes.

Mon excellent collègue M. Thiéry - qui m'écoute, j'en suis sûr -, avait également élaboré un texte sur le sujet. Il a accepté de se joindre à nous. Quelle belle image de voir ainsi réunies l'opposition et la majorité! Il est vrai que la majorité a mis un peu de temps. Elle a été un peu hésitante. Elle a dû se concerter, ne trouvant pas immédiatement l'intérêt de la proposition. Mais finalement, après réflexion et avec sagesse, elle a accepté de travailler sur un texte.

Cela dit, j'ai malheureusement l'impression, au regard des membres présents en séance plénière et vu ceux qui n'écoutent pas, que le sujet intéresse peu.

                    Madame Onkelinx, je sais que vous m'écoutez toujours.

Pourtant, le sujet est extrêmement important dans la mesure où il concerne des milliers de familles dans notre pays. Il faut savoir que 40 % des enfants généralement âgés de dix à quatorze ans reçoivent un traitement orthodontique en Belgique. Le coût de chaque traitement - le montant est interpellant - représente une somme  allant de 500 à 5 000 euros avec, en moyenne, un remboursement de 800 euros de l'assurance soins de santé.

Il faut également rappeler qu'un Belge sur cinq reporte ses soins de santé, ses soins pharmaceutiques, ce principalement pour des raisons de coût, les soins dentaires étant ceux le plus souvent reportés à plus tard. On sait pourtant que ces soins sont essentiels. En effet, certains problèmes dentaires spécifiques ont des conséquences importantes, voire vitales pour la santé des patients. Des avancées sont donc indispensables pour améliorer l'accessibilité aux soins dentaires. Car malgré les interventions de l'assurance soins de santé et les avancées obtenues durant ces dernières années, ces soins restent, dans certains cas, très élevés.

However, the subject is extremely important in that it concerns thousands of families in our country. It should be noted that 40% of children generally aged ten to fourteen years receive orthodontic treatment in Belgium. The cost of each treatment - the amount is appealing - represents a sum ranging from 500 to 5,000 euros with, on average, a refund of 800 euros from the health insurance.

It should also be remembered that one in five Belgians postpones their health care, their pharmaceutical care, this mainly for cost reasons, dental care being the ones most often postponed to later. However, we know that such care is essential. Indeed, certain specific dental problems have significant, even vital, consequences for the health of patients. Advances are therefore essential to improve access to dental care. Because despite the interventions of healthcare insurance and the advances achieved in recent years, these care remains, in some cases, very high.

                    Au sujet des coûts, les raisons sont multiples. Parmi celles-ci, il y a le fait qu'un nombre important de dentistes ne sont pas conventionnés. Les suppléments appliqués sont dès lors importants dans de nombreux cas, en matière de prise en charge orthodontique en particulier. Et le fait aussi que le patient doive avancer l'entièreté des montants avant d'être remboursé par sa mutuelle est un autre obstacle, un frein également.

Monsieur le président, chers collègues, c'est fait! Nous avons à nouveau déposé un amendement qui, malheureusement, a été rejeté en commission et qui vise à mettre en place le tiers payant pour les prestations médicales dispensées chez le dentiste et cela, en priorité pour les bénéficiaires de l'intervention majorée et les jeunes jusque dix-huit ans.

Chers collègues, si vous avez encore un sursaut de sagesse, j'imagine que vous pourrez peut-être voter cet amendement tout à l'heure en séance plénière. Cette partie qui est à charge du patient, qu'il doit débourser, monsieur de Lamotte, en attente du remboursement par les mutuelles, reste évidemment un frein important. Nous verrons donc ce que vous ferez de notre amendement.

Le texte est d'une grande simplicité. Il demande au gouvernement de prévoir un meilleur remboursement des soins orthodontiques pour les enfants, les adolescents mais également les adultes, et un remboursement différencié en fonction de la sévérité des besoins. Il est également demandé au gouvernement d'analyser les raisons du taux de déconventionnement des dentistes et des orthodontistes. Une demande que nous avions formulée et qui a été reprise - merci aux collègues de la commission - était le fait d'obliger les dentistes et les orthodontistes à remettre un devis aux patients, de façon à ce qu'ils sachent à quoi s'attendre.

J'aurais voulu me tourner vers la ministre de la Santé mais, manifestement, le sujet ne l'intéresse pas. Elle est représentée par quatre collègues. Vous lui rapporterez le propos. On a donc trouvé un compromis en commission. Notre concession dans le texte - monsieur Thiéry, vous vous en rendez bien compte -, c'est la phrase ajoutée: "Dans les limites des possibilités budgétaires existantes." Cela peut apparaître comme évident mais cela ne doit, en aucun cas, être un argument pour nous envoyer balader demain. Nous sommes un peu méfiants, voyez-vous, chers collègues du gouvernement.

On a entendu la ministre dire: "Jamais, on ne touchera à mon budget des soins de santé." On a vu les mesures: 900 millions d'euros! C'est un tiers de l'effort budgétaire, monsieur Laaouej, sur le dos des malades et des patients!

When it comes to costs, the reasons are multiple. Among them is the fact that a large number of dentists are not conventional. The supplements applied are therefore important in many cases, especially when it comes to orthodontic care. And also the fact that the patient must advance the full amounts before being refunded by his mutual is another obstacle, a brake as well.

Dear colleagues, this is done! We again submitted an amendment which, unfortunately, was rejected in the committee and which aims to establish the third paying for the medical benefits provided by the dentist and this, in priority for the beneficiaries of the increased intervention and young people up to eighteen years.

Dear colleagues, if you still have a surge of wisdom, I imagine that you may be able to vote on this amendment right now in the plenary session. This part that is borne by the patient, which he must pay, Mr. de Lamotte, waiting for reimbursement by the mutual, remains obviously an important brake. We will then see what you will do with our amendment.

The text is of great simplicity. It calls on the government to provide for better reimbursement of orthodontic care for children, adolescents but also adults, and differentiated reimbursement according to the severity of needs. The government is also asked to analyze the reasons for the rate of discontinuation of dentists and orthodontists. One request that we had formulated and which was taken back – thanks to the colleagues in the commission – was to force dentists and orthodontists to give a quote to patients, so that they know what to expect.

I would have liked to address the Minister of Health but, obviously, the subject does not interest her. It is represented by four members. You will report it to him. There was a compromise in the committee. Our concession in the text – Mr. Thiéry, you know it well – is the phrase added: “In the limits of the existing budgetary possibilities.” We are somewhat suspicious, you see, dear colleagues of the government.

We heard the minister say, "Never, we will never touch my healthcare budget."We saw the measures: 900 million euros! This is a third of the budget effort, Mr. Laaouej, on the back of the sick and patients!

                    Monsieur le président, messieurs les ministres, monsieur Frédéric, chers collègues, permettez-moi d'abord de remercier absolument tous les collègues de la commission Santé publique qui ont travaillé de près ou de loin sur cette proposition de résolution.

Je remercie spécifiquement M. Frédéric pour le texte qui a été présenté par le PS et duquel nous nous sommes également inspirés. Vous aurez constaté qu'un certain nombre d'amendements qui avaient été proposés par le PS et d'autres partis ont été étudiés. Cela nous a permis d'arriver à un consensus sur une prise de charge de l'orthodontie qui nous semblait nécessaire.

Je ne vais pas rappeler les chiffres. Pour éviter les répétitions, je fais référence à l'état des lieux dressé par M. Frédéric.

Je voudrais simplement confirmer que, jusqu'à présent, la prise en charge ne concernait que les enfants jusqu'à quinze ans. Dans des cas exceptionnels, cette prise en charge intervient pour de jeunes adultes jusque vingt-deux ans. Nous savons que certains problèmes dentaires chez les jeunes peuvent nécessiter un certain nombre de soins ou d'interventions bien au-delà de dix-huit ans, voire vingt-deux ans.

Nous savons aussi que les traitements orthodontiques font l'objet d'un remboursement spécifique par les assurances mais sur ce point également, tout le monde n'est pas sur le même pied, entre autres les patients qui n'auraient pas accès à ces assurances.

I would like to thank Mr. Specifically. Frédéric for the text that was presented by the PS and from which we were also inspired. You will have seen that a number of amendments that had been proposed by the PS and other parties have been studied. This allowed us to reach a consensus on taking over the orthodontics that we considered necessary.

I will not mention the numbers. In order to avoid repetitions, I refer to the state of the places drawn up by mr. by Frédéric.

I would just like to confirm that, so far, care was only for children up to fifteen years old. In exceptional cases, this care takes place for young adults up to twenty-two years of age. We know that certain dental problems in young people may require a number of care or interventions well beyond the age of eighteen, or even twenty-two.

We also know that orthodontic treatments are subject to a specific reimbursement by insurance but also on this point, not everyone is on the same foot, including patients who would not have access to these insurance.

                    Il fallait également envisager la prise en charge des patients souffrant d'une pathologie plus sévère, comme la fente palatine, et d'autres patients atteints d'affections médicales pouvant présenter des soucis après l'âge de quinze ans. C'est donc l'une des raisons pour lesquelles nous avons voulu réaliser cette résolution en tenant compte bien entendu aussi de la problématique des prix. Je pense que M. Frédéric l'a rappelé. Un patient devrait pouvoir se présenter chez son orthodontiste en lui demandant à l'avance une estimation des coûts par rapport à l'intervention qui doit être faite. Je pense que nous sommes tout à fait sur la même longueur d'ondes à ce sujet-là.

Il est vrai qu'une résolution, c'est un engagement, mais elle n'a pas force de loi. Certains pourront dire qu'on ne va peut-être pas assez loin dans le cadre de cette résolution mais, encore une fois, si on devait attendre la perfection d'une résolution ou d'un projet de loi, par définition, on sait très bien qu'ils n'aboutiraient jamais. Je n'ai jamais vu de résolution ni de projet de loi parfaits.

Il est évident aussi qu'un certain nombre d'intervenants vont devoir prendre position. Je pense entre autres aux acteurs de la Dentomut qui vont devoir, dans le cadre de la problématique budgétaire, prendre position. C'est la raison pour laquelle nous pensons qu'entre autres, pour tout ce qui touche à la nomenclature, ce n'était pas à nous, dans le cadre de cette résolution, à prendre position. Une concertation devra encore avoir lieu. Nous espérons qu'elle se fera bien entendu toujours dans l'intérêt des patients.

Il importe également de tenir compte du fait que les enfants de plus de quinze ans, mais aussi les adultes qui présentent une nécessité de traitement d'ordre médical ou dentaire, soient suivis. Les traitements orthodontiques pour les enfants atteints de fente palatine méritent également une attention spécifique.

Monsieur le président, chers collègues, voilà les points que je voulais mettre en avant, en étant convaincu que des initiatives concrètes doivent avoir lieu dans le domaine. Je voudrais terminer en signalant qu'il y a eu un amendement proposé par le PS dans le cadre – monsieur Frédéric l'a rappelé – du tiers payant. Nous en avons discuté en commission. C'est vrai que nous n'avons pas abouti à un accord. Nous sommes partis du principe que la problématique du tiers payant était beaucoup plus globale que le cas spécifique, ici, de l'orthodontie. C'est la raison pour laquelle nous n'avons pas suivi cet amendement.

Pour le reste, je voudrais rappeler que beaucoup d'autres amendements ont été pris en considération et ont permis, en commission, d'arriver à un vote à l'unanimité sur cette proposition de résolution.

Treatment should also be considered for patients with a more severe pathology, such as palatine fissure, and other patients with medical conditions that may have concerns after the age of fifteen. This is one of the reasons why we wanted to implement this resolution, taking into account, of course, also the problem of pricing. I think mr. Frederick reminded him. A patient should be able to appear to his Orthodontist by asking him in advance for an estimate of the costs in relation to the intervention to be performed. I think we are on the same wavelength on this subject.

It is true that a resolution is a commitment, but it has no force of law. Some may say that we may not go far enough within the framework of this resolution but, once again, if we should wait for the perfection of a resolution or a bill, by definition, we know very well that they would never succeed. I have never seen a perfect resolution or bill.

It is also clear that a number of stakeholders will have to take a position. I think, among other things, of the Dentomut actors who will have to take a position in the context of the budgetary problem. This is why we believe that, among other things, for everything related to the nomenclature, it was not up to us, within the framework of this resolution, to take a position. There will still have to be a consultation. We hope that this will always be in the best interests of patients.

It is also important to take into account the fact that children over fifteen years old, but also adults who have a need for medical or dental treatment, are followed. Orthodontic treatments for children with palatine fissure also deserve special attention.

Mr. Speaker, dear colleagues, these are the points I wanted to highlight, being convinced that concrete initiatives need to take place in the field. I would like to conclude by pointing out that there was an amendment proposed by the PS in the framework – Mr. Frédéric recalled – of the third paying. We discussed this in the committee. It is true that we have not reached an agreement. We started from the principle that the problem of the third-party payer was much more comprehensive than the specific case, here, of orthodontics. That is why we did not follow this amendment.

For the rest, I would like to recall that many other amendments have been taken into account and have enabled, in committees, to reach a unanimous vote on this draft resolution.
