Monsieur le président, chers collègues, le projet de résolution qui nous est soumis aujourd'hui et qui demande un plan d'action en faveur de l'enfant hospitalisé se veut complémentaire au texte présenté, il y a deux semaines, par notre collègue Florence Reuter sur les soins pédiatriques à domicile.
Les deux textes trouvent d'ailleurs leur origine dans la Charte de l'enfant hospitalisé. Sans reprendre ici l'ensemble des points contenus dans cette charte, il me paraît essentiel d'en souligner quelques-uns:
- les enfants ont le droit d'avoir leurs parents auprès d'eux;
- les enfants et leurs parents ont le droit d'être informés;
- les enfants ont le droit d'être soignés avec des enfants du même groupe d'âge;
- les enfants ont le droit d'avoir la possibilité de jouer, de s'amuser et de bénéficier d'un enseignement normal;
- les enfants ont le droit d'être soignés par du personnel spécialement formé pour cela.
Au cours de nos travaux et de nos auditions, il est clairement apparu que la Charte de l'enfant hospitalisé n'est pas appliquée de la même manière partout dans nos hôpitaux et que les moyens mis à disposition pour y arriver sont encore clairement insuffisants. Une évaluation précise de l'application de cette Charte dans les hôpitaux belges est donc nécessaire et devra être réalisée avec la collaboration des entités fédérées.
Par ailleurs, l'étude réalisée par le professeur De Wever et son équipe sur la spécialité en pédiatrie et son avenir a clairement démontré que les médecins pédiatres étaient les praticiens spécialisés dont les revenus étaient les plus bas et qu'il fallait impérativement revaloriser leurs prestations, tant à l'hôpital, via les honoraires de surveillance, qu'aux consultations et en particulier en hôpital de jour et en salle d'urgence.
Faute d'une telle prise en considération de cette spécialité médicale, nous continuerons à connaître de grandes difficultés de recrutement de pédiatres dans nos hôpitaux.
Si elle est riche en enseignements relatifs à la vision du corps médical et des pédiatres en particulier, cette étude n'avait pas pour objet d'analyser parallèlement les besoins en soins hospitaliers pédiatriques. Aussi, afin de compléter objectivement notre observation sur cette discipline médicale, nous demandons au gouvernement de faire actualiser l'étude Cannoodt-Casaer, réalisée à la KUL en 2002 sur l'appréhension de la pédiatrie du point de vue des patients et de leur famille, l'adaptation éventuelle des normes d'agrément, les critères de programmation des lits hospitaliers et de pédiatrie ainsi que sur les règles de financement adaptées à cette discipline médicale.

Mr. Speaker, dear colleagues, the draft resolution that is submitted to us today, calling for an action plan in favour of the hospitalized child, is intended to complement the text presented two weeks ago by our colleague Florence Reuter on pediatric care at home.
Both texts find their origin in the Charter of the Hospitalized Child. Without referring here to all the points contained in this Charter, it seems to me essential to highlight a few of them:
Children have the right to have their parents with them.
Children and their parents have the right to be informed.
Children have the right to be cared for with children of the same age group.
Children have the right to have the opportunity to play, to have fun and to enjoy normal education.
Children have the right to be cared for by staff specially trained for this purpose.
During our work and our hearings, it has become clear that the Charter of the Hospitalized Child is not applied in the same way everywhere in our hospitals and that the means available to do so are still clearly insufficient. A precise assessment of the application of this Charter in Belgian hospitals is therefore necessary and must be carried out with the collaboration of the federal entities.
Furthermore, the study conducted by Professor De Wever and his team on the specialty in pediatrics and its future clearly demonstrated that pediatric doctors were the specialized practitioners with the lowest incomes and that their benefits must imperatively be revalued, both in the hospital, through surveillance fees, consultations and in particular in the day hospital and emergency room.
Without such consideration of this medical specialty, we will continue to experience great difficulties in recruiting pediatricians in our hospitals.
While it is rich in teachings relating to the vision of the medical body and pediatricians in particular, this study was not intended to parallelly analyze the needs for pediatric hospital care. Also, in order to objectively complement our observation on this medical discipline, we ask the Government to update the Cannoodt-Casaer study, conducted at the KUL in 2002 on the acceptance of pediatrics from the perspective of patients and their families, the possible adaptation of the standards of approval, the criteria for programming of hospital and pediatric beds as well as on the funding rules adapted to this medical discipline.

Mijnheer de voorzitter, ik wil, eerst en vooral, onze verslaggever bedanken.
Vervolgens, om de heer Annemans een plezier te doen, leggen wij alweer een resolutie voor aan de Kamer. Ik herinner mij goed uw woorden, mijnheer Annemans. U zei dat dat allemaal zaken zijn waar men niet tegen kan zijn. Daarom heb ik daarnet aan uw collega gevraagd of u misschien, voor de verandering, zou vóór stemmen en hij heeft mij dat bevestigd.
Collega’s, hoe universeel de rechten van het kind ook zijn – ze zijn sedert een tiental jaar erkend –, in de praktijk zijn ze nog altijd niet helemaal ingevuld. Het gebeurt stap voor stap. Ik vraag uw aandacht voor de resolutie, maar ik zal niet alles herhalen wat de collega’s Otlet en della Faille reeds hebben gezegd. De rechten van het kind zijn niet zo eenvoudig in te vullen. Er is het EACH Charter. Dat is een charter dat Europees werd goedgekeurd en waarin de rechten van kinderen in het ziekenhuis duidelijk worden uitgetekend.
In de resolutie vragen wij een evaluatie. Ik meen dat het rapport nog heel veel hiaten zal aantonen, als de administratie een evaluatie moet maken van de rechten van het kind in de ziekenhuizen. Dat is ook in ons land het geval. Wij hebben nochtans een traditie ter zake en wij zijn gezegend met een goede gezondheidszorg. Wij vragen dus een evaluatie. Het is aan ons, aan het Parlement, indien de tekst wordt goedgekeurd, om het dossier uiteraard ook op te volgen. Het is aan de regering en aan de minister om hiervoor in de nodige middelen te voorzien.
Er zijn al veel stappen gedaan. Sedert 2006 is er het zorgprogramma pediatrie, waarin een aantal kwaliteitsvereisten werd opgenomen. Het was duidelijk, zowel tijdens de bespreking als tijdens de hoorzitting, dat een aantal van die elementen helemaal niet is ingevuld of niet invulbaar is. Zo vraagt men onder meer in het zorgprogramma pediatrie aandacht voor de pediatrisch verpleegkundigen. Wie het terrein kent, weet dat de pediatrisch verpleegkundigen vandaag nog altijd niet de volle erkenning krijgen en zeker nog niet voldoende in aantal zijn. De psychologische ondersteuning en de educatieve mogelijkheden zijn elementen die niet alleen theoretisch mogen ingevuld worden, maar effectief ook in de praktijk.

Colleagues, however universal the rights of the child – they have been recognised for a decade – in practice they are still not fully fulfilled. It happens step by step. I ask your attention for the resolution, but I will not repeat everything that colleagues Otlet and della Faille have already said. Children’s rights are not so easy to fulfill. There is each Charter. This is a charter that has been approved by the European Union and which clearly sets out the rights of children in the hospital.
In the resolution, we ask for an evaluation. I think the report will show a lot of gaps if the administration needs to evaluate the rights of the child in hospitals. This is also the case in our country. However, we have a tradition in this area and we are blessed with good health care. We ask for an evaluation. It is up to us, to the Parliament, if the text is approved, to follow up on the dossier, of course. It is up to the Government and the Minister to provide the necessary resources for this.
Many steps have already been taken. Since 2006, there has been the care program pediatrics, which included a number of quality requirements. It was clear, both during the discussion and during the hearing, that some of those elements were not completed at all or cannot be completed. For instance, in the care program pediatrics calls for attention to the pediatric nurses. Those who know the field know that the pediatric nurses today still do not receive full recognition and ⁇ are not yet sufficient in number. Psychological support and educational capabilities are elements that must be completed not only theoretically, but also effectively in practice.

Het tekort aan pediaters is zowel door collega della Faille als door collega Otlet reeds uitvoerig toegelicht, maar het zal nog schrijnender zijn indien de normen die zijn vooropgesteld tot op vandaag… Ik weet dat enkele collega’s in ons Parlement ook betrokken zijn en ook zoeken naar oplossingen, enerzijds om aan de kwaliteitsnormen te kunnen beantwoorden, maar anderzijds om toch hun afdeling leefbaar te houden en om voldoende mensen te kunnen inzetten. Vandaar dat het tekort aan ziekenhuispediaters absoluut in de resolutie is blijven staan, hoewel aanvankelijk werd gedacht dat dit niet nodig zou zijn omdat er meer pediaters zijn. Echter, dat groter aantal pediaters werkt minder, want verschillende pediaters hebben minder dan duizend patiëntencontacten per jaar.
De oplijsting binnen het kadaster voor de geneesheren-specialisten is dan ook absoluut nodig.
Professor Alain De Wever heeft heel duidelijk het relatieve inkomstenverlies aangetoond door minder hospitalisaties. Maar, collega’s, als u zou weten hoeveel het toezichthonorarium bedraagt voor een pediater voor een ziek kind na de vierde dag… Welnu, dat is schrijnend. Het is nog geen 10 euro voor een toezicht op een ziek kind.
Ik denk dat eenieder wel kan inzien dat een toezicht op een ziek kind niet op twee minuten gebeurt, of zelfs niet op tien minuten. Dat is niet in tijd te meten. Er moet voldoende kwaliteit gegenereerd kunnen worden.
De compensatie van de verminderde vergoedingen voor toezicht, zou uiteindelijk enkel en alleen – en ook in het belang van het kind – kunnen komen door de officiële invoering van de daghospitalisatie voor kinderen en de voorlopige hospitalisatie met bijkomende toezichthonoraria.
Mijnheer de voorzitter, ik eindig met wat mevrouw della Faille heeft gezegd. Hoe uitgebreid de ondervraging en de bevraging van de Belgische pediaters ook is geweest, het is schrijnend dat er nul komma nul interesse of nul komma nul vraag is geweest naar een toename of naar een ondersteuning van de ziekenhuispediatrie.
Er ligt een resolutie voor die wij binnenkort ook uitvoerig zullen kunnen bespreken in onze commissie. Die resolutie duidt de knelpunten in de kinderpsychiatrie heel duidelijk aan.
Ik ben al veertien jaar parlementair actief. Het is exact veertien jaar geleden dat ik, destijds nog in het Vlaams Parlement, aandacht vroeg voor de kinderpsychiatrie. Ik vroeg toen om een bredere ondersteuning evenals om een betere omlijning van en afstemming op de bijzondere jeugdzorg, toen een bevoegdheid van de Gemeenschappen.
Schrijnend genoeg worden wij nu bijna dagelijks geconfronteerd met nieuwe noden en met nog langere wachtlijsten voor kinderen en jongeren die niet kunnen worden opgenomen. Er is steeds meer nood aan het begrijpen van geweld. Er zijn ook internationale netwerken. Echter, niemand heeft daarvoor al de nodige middelen uitgetrokken.
Het College voor Pediatrie is nog niet daadwerkelijk opgestart, hoewel wij meenden dat het al werkzaam was. Dat is niet het geval. Na het advies van de Belgische Academie voor Kindergeneeskunde over de samenstelling ervan is het absoluut noodzakelijk dat er ook een college voor kinder- en jeugdpsychiatrie komt.
Mijnheer de voorzitter, collega’s, ik dank u voor uw aandacht. Op de agenda van de plenaire vergadering staat vandaag slechts één actiepunt. Eenieder die ooit zijn kind of kleinkind in een ziekenhuis moet laten opnemen en verzorgen, zal echter wel terugdenken aan de positieve steun die daaraan werd gegeven.

The shortage of pediatricians has already been explained in detail by both colleague della Faille and colleague Otlet, but it will be even more striking if the standards that have been set up until today... I know that some colleagues in our Parliament are also involved and also looking for solutions, on the one hand to be able to meet the quality standards, but on the other hand to keep their department alive and to be able to deploy enough people. Hence, the shortage of hospital pediatricians has absolutely remained in the resolution, although initially it was thought that this would not be necessary because there are more pediatricians. However, that greater number of pediatricians works less, because different pediatricians have fewer than a thousand patient contacts per year.
The listing within the cadastre for the doctors-specialists is therefore absolutely necessary.
Professor Alain De Wever has very clearly demonstrated the relative loss of income by fewer hospitalizations. But, colleagues, if you were to know how much the supervision fee for a pediatrician for a sick child after the fourth day... Well, that’s scary. It is still not 10 euros for a supervision of a sick child.
I think everyone can understand that monitoring a sick child does not happen in two minutes, or even in ten minutes. This is not measured in time. It must be able to generate adequate quality.
The compensation for the reduced supervisory fees could ultimately come solely – and also in the best interests of the child – through the official introduction of the daily hospitalization for children and the provisional hospitalization with additional supervisory fees.
Mr. Speaker, I would like to conclude with what Mrs. della Faille said. No matter how extensive the questioning and questioning of the Belgian pediatricians has been, it is shocking that there has been zero komma zero interest or zero komma zero demand for an increase or for a support of the hospital pediatrics.
There is a resolution ahead of us that we will soon be able to discuss extensively in our committee. This resolution clearly shows the difficulties in child psychiatry.
I have been active in parliament for 14 years. It is exactly fourteen years ago that I, at that time still in the Flemish Parliament, called for attention to child psychiatry. I then asked for broader support as well as for a better alignment of and alignment with the special youth care, then a competence of the Communities.
Curiously, we are now almost daily confronted with new needs and with even longer waiting lists for children and young people who cannot be accommodated. There is an increasing need to understand violence. There are also international networks. However, no one has allocated all the necessary resources for this.
The College of Pediatrics has not actually started yet, although we thought it was already working. This is not the case. Following the advice of the Belgian Academy of Pediatrics on its composition, it is absolutely necessary that there is also a college for child and youth psychiatry.
Mr. Speaker, colleagues, I thank you for your attention. There is only one action item on the agenda of the plenary session today. However, anyone who has ever had to have their child or grandson hospitalized and cared for will remember the positive support that was given to them.

Monsieur le président, chers collègues, ce doit être la deuxième ou la troisième fois que nous intervenons en séance plénière à propos de la thématique 'santé au bénéfice des enfants'. Il devrait encore y avoir avant les congés, normalement, une dernière séance.
En effet, la commission de la Santé a pris conscience du paradoxe auquel nous confrontent les soins pédiatriques, à la fois en milieu intra et extrahospitalier. En effet, d'un côté, il y a la volonté de bien s'occuper des enfants, de leur consacrer beaucoup de moyens, de leur fournir les personnes et les services les plus adaptés et compétents mais, d'un autre côté, nous devons être conscients de nos limites lorsqu'il s'agit de concrétiser ces objectifs vu les nécessités en infrastructures, personnel et structures à mettre en place en interne par les hôpitaux pour organiser les services et répondre aux préoccupations de ceux qui veulent défendre la santé et les droits de ces enfants.
Le travail que nous avons entamé aura logiquement des conséquences lourdes à assumer, mais pas impossibles, nécessitant une volonté politique exprimée régulièrement par la commission. La commission tient à affirmer l'importance de s'accorder les moyens pour rencontrer nos obligations en termes de droits découlant de la ratification des conventions visant au respect des droits des patients, donc aussi des enfants. Nous aurons donc à adapter les budgets, mais également les pratiques, quitte à fonctionner peut-être autrement.
Un autre élément mis en évidence au cours de nos travaux est la nécessité de repenser le travail en hôpital en lien avec ce qu'il est possible de réaliser à domicile, en soins de proximité; il conviendra sans doute de répartir autrement les moyens à la fois financiers, de compétences et de prestataires de soins.
En ce qui concerne cette résolution relative aux soins hospitaliers à destination des enfants, je tenais à intervenir spécifiquement aujourd'hui pour faire part, comme Mme Avontroodt, de mon étonnement d'avoir entendu le professeur De Wever s'exprimer à propos de l'étude sur les pédiatres, commandée par les pédiatres: bien qu'elle ait été réalisée dans ce contexte, elle n'a pas posé la question de savoir si les pédiatres ont une place reconnue utile complémentaire dans l'ensemble des prestataires de soins de manière à présenter une offre médicale capable de rencontrer au mieux les besoins.
Nous avions déjà connaissance de la pénurie de médecins choisissant la spécialisation de pédiatres. Elle est surtout visible au sein des hôpitaux et résulte du manque de valorisation des prestations des pédiatres, et notamment de leur obligation de remplir une fonction de garde dans les hôpitaux au bénéfice des enfants. Ce problème est identifié depuis longtemps, et nous savons qu'il convient d'y remédier. Mais il est aussi lié au développement de prestations de certains cabinets extérieurs aux hôpitaux, dans lesquels le pédiatre s'imagine de plus en plus comme étant un médecin généraliste au service des enfants.
Comment envisage-t-on le rôle des spécialistes en pédiatrie? Doivent-ils intervenir pour soigner des pathologies qui sont particulièrement difficiles et dont certains enfants sont porteurs? Ou bien considère-t-on qu'un patient enfant doit automatiquement recourir aux services d'un pédiatre?
L'étude en question montre aussi que ces pédiatres s'imaginent dans le rôle du généraliste pour enfants, mais refusent d'en assumer les aspects désagréables, c'est-à-dire assurer des gardes, donc travailler après 18.00 heures et durant le week-end.
Nous nous trouvons devant une tâche d'une plus grande ampleur qui devrait aussi débuter au sein de la commission et qui consistera à réexaminer l'offre médicale. Il s'agira de voir quel rôle pourrait remplir le politique dans la mise en place d'outils de régulation de l'offre médicale qui seraient plus performants et qui tiendraient compte des besoins des patients, de l'évolution des pathologies et d'une combinaison indispensable entre les milieux intra et extrahospitaliers, de manière à ce que ces acteurs de santé soient complémentaires et non pas concurrents.
Enfin, je me permettrai d'insister sur une dimension de cette résolution que nous approfondirons bientôt en commission et qui a trait aux services et soins de pédopsychiatrie. Nous savons qu'il est nécessaire d'améliorer les services et l'infrastructure, les compétences du personnel à destination des enfants, l'intra et l'extrahospitalier, le respect des droit des patients, des enfants et des parents – qui sont parfois contradictoires dans les faits. À cette fin, nous devrons revoir la régulation de l'offre et l'organisation des soins au bénéfice des enfants.
Madame Avontroodt, je tenais à vous remercier pour votre résolution que nous allons soutenir en sachant qu'elle s'intègre dans un ensemble de résolutions que nous adressons au gouvernement et qui vont demander des moyens et de la volonté politique.

This should be the second or third time we speak in a plenary session on the theme of "health for the benefit of children". There should still be before the holidays, normally, a last session.
Indeed, the Health Commission has become aware of the paradox facing pediatric care, both in the intra and extra-hospital environment. Indeed, on the one hand, there is a willingness to take good care of children, to devote many resources to them, to provide them with the most suitable and competent people and services but, on the other hand, we must be aware of our limits when it comes to realizing these goals given the needs of infrastructure, personnel and structures to be set up internally by hospitals to organize services and address the concerns of those who want to defend the health and rights of these children.
The work we have begun will logically have heavy consequences to assume, but not impossible, requiring a political will regularly expressed by the commission. The committee wishes to affirm the importance of giving each other the means to meet our obligations in terms of rights arising from the ratification of conventions aimed at respecting the rights of patients, therefore also children. We will therefore have to adjust the budgets, but also the practices, even if it might work otherwise.
Another element highlighted during our work is the need to rethink hospital work in connection with what is possible to accomplish at home, in nearby care; it will probably be appropriate to distribute the resources both financial, skills and care providers in a different way.
With regard to this resolution on hospital care for children, I wanted to intervene specifically today to speak, like Ms Avontroodt, of my amazement to have heard Professor De Wever speak about the study on pediatricians, commissioned by pediatricians: although it was carried out in this context, she did not raise the question of whether pediatricians have a recognised additional useful place in all the providers of care in order to present a medical offer that best meets the needs.
We were already aware of the shortage of doctors choosing the specialization of pediatricians. It is mostly visible in hospitals and is the result of the lack of appreciation of the services of pediatricians, and in particular of their obligation to fulfill a custody function in hospitals for the benefit of children. This problem has been identified for a long time, and we know that it needs to be addressed. But it is also related to the development of services of certain offices outside hospitals, in which the pediatrician increasingly imagines himself as a general physician in the service of children.
What is the role of pediatric specialists? Should they intervene to treat pathologies that are ⁇ difficult and that some children are carriers of? Or is it considered that a child patient should automatically use the services of a pediatrician?
The study in question also shows that these pediatricians imagine themselves in the role of the generalist for children, but refuse to assume the unpleasant aspects, that is, to provide guards, so work after 18:00 and during the weekend.
We are facing a larger task that should also start within the committee and which will consist of reviewing the medical offer. It will be a question of what role the policy could play in establishing tools for regulating the supply of medical care that would be more efficient and that would take into account the needs of patients, the evolution of pathologies and an indispensable combination between the intra- and extra-hospital environments, so that these health actors are complementary and not competing.
Finally, I will allow myself to insist on one dimension of this resolution that we will soon deepen in committee and which relates to the services and care of pediatric psychiatry. We know that there is a need to improve services and infrastructure, the skills of staff for children, the intra and extra-hospital, the respect for the rights of patients, children and parents – which are sometimes contradictory in the facts. To this end, we will need to review the regulation of supply and the organization of child care for the benefit of children.
Madame Avontroodt, I wanted to thank you for your resolution that we will support knowing that it is part of a set of resolutions that we address to the government and that will require means and political will.
